Your Time is Valuable

At your first visit, you will be greeted by one of our Patient Account Specialists. They will walk you through your registration process. You will be given a notice of privacy practices NOTICE OF PRIVACY PRACTICES as well as your clinical paperwork to fill out regarding your specific reason for treatment, if not already filled out prior to your appointment time.  You can save some time during your first visit by completing your intake paperwork before you arrive.

In addition to helping you complete your registration process, your Patient Account Specialist will assist you in scheduling your therapy visits, reviewing your insurance benefits, cost of care plan and settling any payments that you may need to make. They can also help you with any other non-clinical aspects of your care. Please look to them as a resource to help you achieve a great outcome during your time in therapy.

Initial Evaluation

Each patient will meet with a Licensed Physical Therapist for a one-on-one evaluation during his or her first visit. They will correspond with your physician after the evaluation and send updates to your physician as you progress in therapy. Please keep us advised of any follow up appointments that you may have with your physician.

During your evaluation your Physical Therapist will discuss the history of your present injury, review your past medical history, and perform tests to determine the best course of action for your treatment.

Following your initial evaluation you will be scheduled for 1-3 times per week, as needed, in order to reach your treatment goals. Our Patient Account Specialist will assist you with your schedule to assure the most convenient times that work for you.

What to Bring

During your first visit you should bring:

  • Physical Therapy Orders/Prescription, given to you by your referring physician

  • Medical Insurance ID Card and Photo ID

  • List of Current Medications/Prescriptions

  • Reports of Tests or Imaging – such as X-rays or MRIs

  • To save time, bring your completed paperwork.

Wear comfortable clothes such as sweats or shorts and t-shirt, depending upon the body part being treated.  A changing area is available for your convenience.
